INTRODUCTION The cost of health care has increased rapidly over the last twenty years. A major factor contributing to this increase has been the great advance made in medical knowledge, bringing with it an increasing range of laboratory tests. The speed of development has had various consequences; tests of little proven value and>tests which have been superseded by better tests continue to be requested by force of habit, duplicate tests providing similar information may be requested on the same patient and the tendency to order multiple investigations because they are available increases. This inappropriate use of tests increases laboratory costs without benefit to the patient. Hence the identification of the most appropriate laboratory tests (essential tests) would have important clinical and economic consequences and assist in improving health care in the community as a whole. Although this document addresses itself primarily to the ! problems of the developing world, the principles outlined here are also highly relevant to hospitals in the industrialized countries. The term essential cannot be defined on a scientiflc basts. In theory any one test of several hundred different known tests may be argued to be essential in a given clinical situation for optimal management of a patient. Purthermore, tests may be essential in a given geographic area depending on local disease patterns, and the socio-economic conditions. h practice, however, the availability of laboratory services are always restricted by limitations of local resources and this important factor has played a major part in deciding which tests should be included in the list of essential tests (see 5.1 Selection criteria, page 3).
