Excerpt
Adequate methods of c l a s s i f y i n g p a t i e n t s into meaningful groups are necessary for comparing large numbers of p a t i e n t s or f o r studying the incidence, etiology, treatment, and outcome of d i f f e r e n t disorders. Clinical diagnosis i s one c l a s s i f i c a t i o n method, but s i n c e d i a g n o s t i c systems and c r i t e r i a for diagnosis vary, and since no one system has been d e f i n i t e l y validated, improved methods for c l a s s i f y i n g p a t i e n t s need to be developed. The development of c l i n i c a l l y more useful c l a s s i f i c a t i o n s of p a t i e n t s can be approached i n several ways. One of these ways i s described in Chapt e r 1 and concerns WHO'S programme on standardization of psychiatric diagnos i s , c l a s s i f i c a t i o n , and s t a t i s t i c s . Another i s the computer-simulated diagnosis discussed in Chapter 11. It i s possible, however, t h a t current d i a g n o s t i c c a t e g o r i e s , even i f combined and standardized, can be improved upon and t h a t other methods of c l a s s i f y i n g p a t i e n t s might yield groups with a common etiology, course, and response to treatment. For t h i s reason, a third method for identifying groups of p a t i e n t s has been attempted: c l u s t e r analysis, an empirical method. A noteworthy feature of t h i s type of methof i s t h a t , once items are selected and ratings made, cases are c l a s s i f i e d e n t i r e l y on the basis of s t a t i s t i c a l c r i t e r i a of t h e i r s i m i l a r i t y . In the process of clustering, each item i s given a weight equal to t h a t of the others. Aside from the decision of which c r i t e r i a of s i m i l a r i t y should be embodied i n the c l u s t e r i n g
