It is a worldwide observation that health and welfare systems are facing growing difficulties in reaching out to entire populations, and that there is a need to learn from the lessons of the diverse systems in the East, West, North and South. At the same time, there is encouraging evidence of growing recognition of the importance of the interrelationship of health and welfare systems in achieving improved health and well-being for all. Coupled with this, people's expectations of health and welfare services are increasing, the role of governments is rapidly changing and the private sector and civil society are emerging as important players. In the 21St century there will be a transition from a state of dependency on systems, to one based on empowerment of peopIe to take more responsibility for their own personal health and well-being, as well as those of their community. Strategies for population-based health reform will have a greater and more sustainable impact if they are part of strong evidence-based people-oriented systems. Health and welfare systems will, therefore, need to go through a transitional phase in order to support these efforts in an appropriate manner. To do so, professionals in the health and related sectors will need to be more socially concerned and involve the people more closely in developing systems and implementing services.
