1. BACKGROUND The peak incidence of rheumatic f ever/rlleumatic heart d l s e a s e (RF/KHD) is i n c h i l d r e n betwedn 5 and 15 years of age; RF/RHD is most prevalent i n developing c o u n t r i e s and i n t h e i n n e r c i t y a r e a s of some i n d u s t r i a l i z e d c o u n t r i e s . Prevalence r a t e a of 2-15 per 1000 have been reported i n school-age c h i l d r e n of urban slums i n some developing c o u n t r i e s . In developing c o u n t r i e s , RHD accounts f o r over 30 per cent of c a r d l a c cases admitted t o h o s p i t a l . General medical treatment is only symptomatic o r supportive and the dramalic advances i n c a r d i a c surgery have not g r e a t l y reduced e i t h e r morbidity o r m o r t a l i t y .
